Prison and internment in Switzerland

In Switzerland, people who have committed very serious crimes can be sentenced to prison followed by internment. But there is also what is sometimes referred to as "petit internnement", i.e. an obligation of care for offenders who suffer from psychological problems. Due to a lack of places in dedicated structures, the vast majority of people sentenced to institutional treatment end up in prison. How does this system work? What are its consequences and limitations?
